How to use the ErinoakKids Online Referral system:

  • To access the online referral forms, please review the Terms and Conditions below by selecting the three categories (Privacy, Disclaimer and Contact). To accept these Terms and Conditions, please check the box to the right, at which point the referral options will display.

  • For school board associates who would like to make a referral to School Based Rehabilitation Services (SBRS) for children attending school in Peel Region, Halton Region, and Dufferin County, please select Start a new referral to SBRS.

  • For families, physicians, and community partners who would like to make a referral to ErinoakKids’ services, please select Start a new general referral.

  • For families, service providers, and community partners who would like to make a referral to the Ontario Autism Program’s Urgent Response Service (OAP URS) for children and youth residing in the Peel Region, Halton Region, Wellington County, Dufferin County, and Waterloo Region, please select Start a new referral to OAP URS.

  • For families to refer a preschool age child for speech and language services, please complete a new communication checkup for ErinoakKids here.

 

In order to refer a client to ErinoakKids, you must have written or verbal consent from the legal guardian of the child or youth, or the youth themselves (if they are 16 years of age or older).

 

We are committed to keeping your information safe and confidential. We follow the rules set out in law about collecting, using and disclosing your personal information. To ensure the protection of your personal information, we have:

  • Privacy policies publicly posted on the ErinoakKids website.
  • Appointed a Privacy Officer to answer your concerns or inquiries.
  • Set up safeguards to protect how your information is stored in our database and in the handling of paper files, electronic and verbal personal information.

The personal information you have provided to us will be sent securely to ErinoakKids intake department. Once ErinoakKids has obtained the information, it will be securely stored in your child’s medical record.

All of the content contained on ErinoakKids website is edited, checked, and verified for accuracy as much as it is possible to do so. However, we cannot guarantee either its accuracy or the safety of any external links it might contain. The ErinoakKids website, as well as its owners, affiliates and contributing authors can therefore not be held responsible for any problems or damage that occurs as a result of making use of material contained on our site.
